New Patients

We ask new patients to arrive approximately 15 minutes earlier than their scheduled appointment to complete necessary paperwork. You may complete these forms in advance by clicking here (link to forms). Please bring the completed forms, a list of medications you currently are taking, your insurance card and any recent copies of X-rays or lab results with you to your appointment.

Billing & Insurance

Any co-payment and past-due account balances are collected at the time of services. We accept cash, checks, debit and all major credit cards.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

Eastside Medical Group accepts most major insurance plans. We recommend you call your insurance company to verify that we are in your plan’s network of approved providers. Please bring your insurance card and a form of identification with you to each visit and be sure to notify us if there is any change in your name, contact information or insurance as soon as the change occurs.

Cancellations or Rescheduling

If there has been a change in your schedule or availability, do not worry. We simply ask that you call 24 or more hours in advance to let us know and we’ll gladly reschedule your appointment.
